Help us to recognize deserving physical therapists and physical therapist assistants working in Montana. The Chapter presents several awards on an as-needed basis. This document includes the criteria for each award.
Member Survey
The survey allows you to tell us a little about yourself and your interests with regard to services from the Chapter. You can also use the survey to provide us with information for your directory listing and volunteer for Chapter Committees.
Peer Reviewer Application
Interested in working with the Chapter to conduct Peer Review? Please let us know by submitting an application. The Chapter typically provides peer review training to its peer reviewers.

MAPTA offers three membership scholarships annually to APTA/MAPTA members experiencing financial hardship. We want you to be able to continue your membership in the Association. This document includes the criteria and application.
CEU Endorsement Application
You may apply for CEU endorsement for continuing education programs appropriate for physical therapists and physical therapist assistants through the Montana Chapter. The endorsement ensures that the Montana Board of Physical Therapy Examiners will accept the CEUs toward the education requirement of licensure renewal in Montana. We prefer electronic submission.
